Peptides for longevity, ranked by evidence
Peptides are one of the most hyped corners of longevity, and one of the most misunderstood. A few are approved medicines with strong trials. Many are research only, sold through grey channels, with little or no human evidence. The trick is telling those apart before you spend money or take a risk.
Rather than a brand list, this is a way to judge any peptide, built on the same method behind the Peptide Evidence Index. Everything here is educational. Therapeutic peptides are prescription or research only and belong with a licensed clinician.
How to choose
- Evidence. Is there real human data, and on hard outcomes or just surrogate markers?
- Regulatory status. Approved, research only, or restricted, across FDA, WADA and the UAE DHA framework.
- Supply integrity. Is there a legitimate, quality controlled source, or is the market unverified?
The options, honestly
GLP-1 medicines: semaglutide and tirzepatide
These are the highest scoring peptides in our Index for a reason. They are FDA-approved, backed by large trials, and show real cardiovascular and metabolic benefit. They are prescription medicines that require clinical supervision, not something to source yourself. See semaglutide and tirzepatide.
Tesamorelin and a few others
A small set of peptides are approved for specific medical uses and carry genuine evidence within those uses. Approval for one thing is not proof for everything, and that distinction matters. Browse the full list in the Peptide Universe.
The research-only crowd (BPC-157, TB-500 and similar)
These are widely sold and widely discussed, but they are not FDA-approved, human evidence is thin or animal only, and supply is largely unregulated. That does not make them useless, it makes them unproven, and anyone selling them as a sure thing is ahead of the science. Each has an honest page, for example BPC-157.
The bottom line
The pattern is uncomfortable but consistent. The peptides with the strongest evidence are prescription medicines you get through a clinician, and the ones with the loudest online marketing tend to be the least proven. Judge any peptide on evidence, regulation and supply before anything else, and take prescription decisions to a licensed clinician.

Are peptides legal?
It depends on the peptide and your country. GLP-1 medicines like semaglutide are approved prescription drugs. Many research peptides are not approved for human use and are legally grey to buy, and in the UAE most therapeutic peptides are prescription only through a DHA-licensed clinic. Are peptides safe?
Approved peptides have known safety profiles when used under medical supervision. Research-only peptides from unverified sources carry real risks, both from the compound and from contamination or mislabeling. Safety and legality are exactly why prescription decisions belong with a clinician.
Which peptides actually have real evidence?
The GLP-1 medicines, semaglutide and tirzepatide, have the strongest human evidence by a wide margin, along with a handful of peptides approved for specific medical uses. Most of the popular research peptides have little or no human outcome data.
